Incident Investigation

Over the past 20 years, we have conducted Hardware-in-the-Loop (HIL) testing on approximately 1,000 control systems—ranging from small hydraulic manifolds to complex drilling control systems. This extensive experience across diverse systems and manufacturers has given us deep insight into control systems, networks, servers, alarm systems, and the human/operator interface—along with the many ways things can go wrong.

Our technical expertise, combined with strong academic backgrounds in control systems and software design, has proven invaluable in incident investigations. We have supported numerous investigations, working alongside multidisciplinary teams of skilled DNV colleagues. Each team can be assembled based on the specific competencies required for the case, drawing from DNV’s broad pool of subject matter experts.

We evaluate the human, technological, and organizational factors related to the incident. Our approach includes:

  • Evidence collection through interviews, data logs, site inspections and so forth
  • Data analysis to identify root causes
  • Documentation of findings and actionable recommendations

As an independent third party, we provide objective insights and support throughout the investigation process.
