Inside failure investigations: How evidence, lab analysis, and independent expertise reveal cause of failure
When a pipeline fails, the consequences can be costly, disruptive, and technically challenging to untangle. Various stakeholders are affected, including landowners, regulators, and operators. This webinar will introduce the investigative steps DNV uses to identify causal factors and determine defensible conclusions.
Join DNV expert Greg Quickel and guest Gary Vervake, Senior Engineer Specialist – Metallurgist at Enbridge to learn how investigation findings are developed, validated and translated into actionable reporting for pipeline integrity teams, regulators and other stakeholders.
Why should you attend?
- Explore failure scene assessment to see how initial evidence collection shapes the direction of an investigation
- Understand metallurgical examination and the role of visual inspection, sectioning, fractography, metallography, and microscopy in identifying damage mechanisms
- See how material and chemical tests confirm or rule out suspected causes
- Gain insight into the structured approach used to reach clear, defensible conclusions for damage cause determination
- Learn how findings are translated into clear, defensible reporting for internal and external stakeholders
Featured speakers
Greg Quickel is a Principal Engineer and Director of Incident Investigation at DNV with 21 years of experience and more than 500 failure analysis projects across the oil and gas, utilities and aerospace industries. His work includes pipeline failure mechanisms such as seam weld defects, corrosion, stress corrosion cracking, fatigue, hydrogen cracking and mechanical damage.
Gary Vervake is a Senior Specialist Metallurgical Engineer at Enbridge with more than 35 years of experience in pipeline integrity, metallurgical failure investigation and materials engineering. He has led investigations of significant in-service pipeline incidents and supports integrity management programs addressing threats such as SCC, hard spots, dents, mechanical damage and construction-related issues.
