Understanding and Using M-SCAT for Marine Incident Investigations

Training course to support users of DNV's M-SCAT tool for root cause analysis of marine incidents.

To enable participants to learn from maritime accidents and near misses and quickly identify the corrective actions necessary to prevent similar events in the future, using DNV’s Marine Systematic Cause Analysis Technique (M-SCAT).

The Marine Systematic Cause Analysis Technique (M-SCAT) is a powerful tool that supports the rigorous investigation of an incident or near miss and ensures that the basic causes of the incident are understood and appropriate management system improvements are identified (controls) to prevent reoccurrence.
The M-SCAT tool is the maritime version of the SCAT which finds its origins in DNV’s loss causation model and the ISRS framework. www.isrs.net. This workshop will help all M-SCAT users to find the right inferences and thus make an accurate root cause analysis of incidents by effectively using the M-SCAT poster and M-SCAT App.


The course focuses on:

  • Introduction to M-SCAT and understanding the M-SCAT Model
  • Evidence and Evidence Collection
  • Understanding the “Barrier” Model (Swiss cheese Model)
  • Analyzing different scenarios through case studies
  • Clarification of common misconceptions
